Minor Release (Notary Form)
Use this form when a parent or guardian cannot physically be present at the track. It must be notarized. When requesting the form be applicable for the entire season, the top line on each side must contain the phrase: for all Kalamazoo Speedway sponsored events (seasonal)
Minor Release (Standard)
This form is used when a parent and guardian WILL be present at the track and must be signed in the presence of a Kalamazoo Speedway staff member. The form may be filled out in advance; however, please do not sign until you are purchasing your pit passes. The top line on each side should contain the phrase noted above.
